Please submit this registration to the captain of your team. New players should send this registration form only if accepted by an existing team captain. Captains must notify the league if they plan on adding players to an existing roster. Minimum age requirement for participation is 40. Players must be 40 or turn 40 during the regular season to be eligible for participation. No Exceptions. New players are required to provide a copy of photo identification with a birth date. Driver's License is preferred. SS # and Driver's # can be crossed out. If a player does not have a driver's license, another form of photo identification with date of birth is required.
