Winter Session 1 Application
 

Winter Session 1 Games Have Begun



Minimum age requirement for participation is 40. Players must be 40 or turn 40 during
the regular season to be eligible for participation. No Exceptions. New players are
required to provide a copy of photo identification with a birth date. Driver's License is
preferred. SS # and Driver's # can be crossed out. If a player does not have a driver's
license, another form of photo identification with date of birth is required.
